Findhelp Launches SCN Partnership for NY Medicaid Waiver

Findhelp, a leader in closed-loop referral platforms, has partnered with three Social Care Networks (SCNs) to enhance the New York Health Equity Reform (NYHER) initiative aimed at integrating health and social care for Medicaid members in New York. Effective from January 1, the partnership involves SOMOS Community Care, Forward Leading IPA, and Western New York Integrated Care Collaborative. Together, they will address the health-related social needs of over 1.5 million Medicaid members. The project involves the use of Findhelp's technology to create a direct connection with New York's Health Information Exchange, enabling secure data exchange and payment facilitation between SCNs, health providers, and community organizations.
